A case of a 17-year-old male was reported to us in December 2002. He ingested an extract of Syrian Rue and then smoked an unknown (but large) dose of powdered, chemical 5-MeO-DMT. He quickly became non-responsive and fell to the ground, began convulsing (myoclonic jerking), and his sitter called an ambulance for help. He was found to have had a pulse of 180, blood pressure 200/125, and temperature 106.0 F. These levels are dangerously high. He was minimally responsive but became wildly agitated upon stimulation (even just speaking to him). He was treated with diazepam and transferred by helicopter to a Boston area hospital. The patient may have suffered lasting damage to his kidneys and rhabdomyolysis. A formal case report of this incident was published in a 2004 issue of the Journal of Toxicology & Clinical Toxicology titled "Monoamine Oxidase Inhibitor Poisoning Resulting from Internet Misinformation on Illicit Substances".

Like other [[serotonergic psychedelics]], 5-MeO-DMT is considered to be [[Addiction potential::non-addictive with a low abuse potential]]. There are no literature reports of successful attempts to train animals to self-administer 5-MeO-DMT — an animal model predictive of abuse liability — indicating that it does not have the necessary pharmacology to either initiate or maintain dependence.

Effekter

Hallucinogen. 5-MeO-DMT är 4-5 gånger potentare än N,N-DMT. Effekten är snarlik gällande intensitet, men med mycket mindre grad av visuella effekter (CEV/OEV). Den kan jämföras med bufotenin, vilket är rimligt med tanke på att den metaboliseras av kroppen till bufotenin (läs mer under kemi).

En studie visar dock att 50 evaporerat extrakt av Bufo alvarius motsvarar 30 mg psilocybin i mätningar av mystiska upplevelser[1]

Effekten kommer snabbt om den röks, ca 10 - 60 sekunder efter inhaleringen. Effekten varar 10-30 minuter. Man blir avdomnad fysiskt, pirrningar i kroppen. Psykiskt kan effekten upplevas starkt euforisk, men välmåendet kan även upplevas som kemiskt och förgiftande. Andra bieffekter kan vara obehag/smärtor i lungor och andningsvägarna. Vid högre doser förekommer fall av kortvarig medvetslöshet (1 - 10 minuter under peaken). Att ha en sitter rekommenderas.

Medicinska effekter

I bl.a Mexiko finns det mer eller mindre seriösa kliniker som erbjuder terapi med sekret från padda.

På den vetenskapliga fronten har det under 2010-talet börjat forskas på 5-MeO-DMT och flera medicinska effekter har framkommit. En studie från 2017 visar att 5-MeO-DMT förändrar hjärnans kemi så att proteiner bildas som har antiinflammatoriska[3] och neuroprotektiva egenskaper, men kan även motverka drogberoende och behandla depressioner[4][5]. 5-MeO-DMT kan även stimulera neurogenes (nybildning av hjärnceller)[6][7]

En enkätundersökning från 2018[8] visar att risken för medicinska eller psykiska bieffekter samt beroende är låg. Majoriteten av deltagarna som hade olika sjukdomar/diagnoser upplevde förbättringar av symptom. PTSD (79%), depression (77%), ångest (69%), alkoholism (66%) eller drogberoende (60%).

Effektiviteten mot depressioner framkommer ännu tydligare i en studie från 2019[9]:

In a survey of 362 adults, approximately 80 percent of respondents reported improvements in anxiety and depression after use. These improvements were related to more intense acute mystical effects during the 5-MeO-DMT experience, as well as increases in rating of the personal meaning and spiritual significance of the experience. Improvements were also related to stronger beliefs that the experience contributed to enduring well-being and life satisfaction.

Och samma forskarlag rapporterar i en annan studie[2] att deltagarna som fick 5-MeO-DMT upplevde minskad depressision, ångest och stress samt ökad nöjdhet med livet, men även se förbättrade kognitiva effekter såsom ökat konvergent tänkande och medvetenhet.

Although 5-MeO-DMT is present in some plants used in ayahuasca and in some ayahuasca brews, Erowid has received several reports of very troubling physical reactions in people who have ingested 5-MeO-DMT with MAOI harmala-alkaloids. There appears to be the risk of severe hypertensive symptoms, overheating, serotonin syndrome, etc. It is important to note that the profile of these risks are not fully understood, because the long history of use of brews containing some amount the two in combination.

A case of a 17-year-old male was reported to us in December 2002. He ingested an extract of Syrian Rue and then smoked an unknown (but large) dose of powdered, chemical 5-MeO-DMT. He quickly became non-responsive and fell to the ground, began convulsing (myoclonic jerking), and his sitter called an ambulance for help. He was found to have had a pulse of 180, blood pressure 200/125, and temperature 106.0 F. These levels are dangerously high. He was minimally responsive but became wildly agitated upon stimulation (even just speaking to him). He was treated with diazepam and transferred by helicopter to a Boston area hospital. The patient may have suffered lasting damage to his kidneys and rhabdomyolysis. A formal case report of this incident was published in a 2004 issue of the Journal of Toxicology & Clinical Toxicology titled "Monoamine Oxidase Inhibitor Poisoning Resulting from Internet Misinformation on Illicit Substances".

In December 2004, we received an unconfirmed report of a death related to the ingestion of 200 mg of 5-MeO-DMT with some amount of "yage". This death was later described in the case report "A Fatal Intoxication Following the Ingestion of 5-Methoxy-N,N-Dimethyltryptamine in an Ayahuasca Preparation" in the November/December 2005 issue of the Journal of Analytical Toxicology, and the follow-up commentary "A Demand for Clarity Regarding a Case Report on the Ingestion of 5-Methoxy-N,N-Dimethyltryptamine in an Ayahuasca Preparation" was published in the July/August 2006 issue.
